PPI Forms
Letter of Authority
This is required so that we can approach the lender on your behalf. This is your authority to allow us to act on your behalf when speaking to your lenders regarding PPI.

- Add your name and signature, and that of any joint borrowers.
- Add the name of PPI seller(s) (this is the name of the company/ies that sold you the PPI product e.g. Barclays or if you bought your product through a broker this will the broker's name)
- If you are claiming for more than one PPI policy you must ensure that you add the name of the seller for each company so we can approach each company on your behalf.
Client Questionnaire
This is required for EACH policy where you have a possible PPI claim i.e. if you have two policies where you believe you have been mis-sold you will need to fill in a questionnaire for each.
This form will allow us to ascertain if there is a case of mis-selling from the lender.

- Answer all the questions as relevant – if there are any questions that you are unsure on please give us a call and we will be able to go through the information with you.
- If there are more than one of the questions that apply to you please ensure that you tick all relevant boxes. This will assist in strengthening your case when we approach the lender on your behalf.
- Enclose where possible the original credit agreement for your loan, credit card or store card and/or 3 months credit or store card statements detailing the PPI.
Terms of Business Agreement
The Terms of Business is our agreement with you; you must read the document and take a copy for your records.
